Know before you start
Before you can request a contract, you must be credentialed. To verify your credentialing status, check our provider directory or contact us.
For faster processing, make sure your CAQH data is current and attested.
You can request an individual contract or join one of our contracted independent practice associations or medical groups.
To establish a contract in Washington, you must primarily provide care in Washington state. You may be eligible if you practice in a county bordering Washington state. Questions? Email us.
Apply for new credentials
How to apply
After meeting our credentialing criteria and standards, you can request an individual contract or join one of our contracted independent practice associations or medical groups. We use the Council for Affordable Quality Healthcare (CAQH) for credentialing applications.
Medical and dental providers
Medical and dental providers: Submit your application by signing in to CAQH.
- Confirm your provider type is on our list of approved specialties.
- Email provider.relations@premera.com if your specialty isn’t listed.
- Sign in to CAQH to complete and attest your provider profile.
- Go to the Authorization tab and select Premera Blue Cross or global for access to review your profile.
- Make sure your CAQH profile has your main email address. Certify will use it to send you application updates.
- Log in to Certify to submit your signed credentialing application.
- Certify will send you an email if additional information is required.

Facility providers
To request a contract, please email us and provide the following information:
- Business legal name
- Doing business as (DBA) name
- Tax identification number (TIN)
- Business specialty
- Business address
Our network team will review your request and follow up with next steps or any additional info needed.

What happens next
You will receive an email confirmation after your request is submitted.
Credentialing is performed through our partner, Certify. Certify may contact you directly if additional information is needed during the credentialing process.
If a contract is requested, our network team will review your request and send contract documents via DocuSign.
The contract is not effective until it is signed and returned and countersigned.
If required documents are not completed within the specified timeframe, your request may be closed.
Credentialing verification
We partner with Certify, a National Committee for Quality Assurance (NCQA)–certified credentialing verification organization, to perform credentialing verification and ongoing monitoring.
Certify may contact you on our behalf during the credentialing process. This partnership helps streamline credentialing and improve processing times.

Use Better Doctor to update your provider information and directory listing.
Request a new contract
To request a new contract, email us the following information:
- Copy of the SS-4 or 147c IRS letter
- DBA name
- TIN
- Address (location, remit/mail; note if for telehealth only)
- Phone number
- Email (for DocuSign)
- Practitioner name(s) and NPI(s)
- Business owner name(s)
- Requested effective date
To establish a contract in Washington, you must provide care primarily in Washington state. You may be eligible if you practice in a county bordering Washington state.
Request a contract copy
To request a copy of your contract, email us with:
- Contract name (legal name as reported to the IRS)
- Tax identification number (TIN)
- Your relationship to the provider
Once received, we’ll send your contract as an email attachment.
