Appointment Scheduling for Complex or Chronic Conditions

Members with suspected chronic or complex conditions who have not visited with a primary care provider in the last 12 months will receive calls from My Advocate. My Advocate will contact the member to offer help in scheduling an appointment, a free service. If interested, the representative will assist with scheduling with the member’s primary care provider.

If the member doesn’t have a provider already selected, My Advocate will assist with finding an appropriate provider that meets their needs.

What this means for you: Your office may receive a three-way phone call from a representative from My Advocate, and the member in need of an appointment, requesting to schedule an appointment. Once the appointment is scheduled, your office will receive a fax confirmation including date and time.

Starting the week of July 22nd, My Advocate will begin conducting outreach to providers for this program and will continue making appointments through August 23rd, 2019.
