Care Gap Letters or Member Scorecard
In September, select members with a specific need for the information will receive:
- A letter about the cancer screening that’s due, if they’re due for only ONE of the following:
- Breast Cancer Screening
- Cervical Cancer Screening
- A scorecard if they’re due for two or more of the following:
- Diabetic HbA1c Testing
- Breast Cancer Screening
- Cervical Cancer Screening
- Colorectal Cancer Screening
OR
The scorecard has personalized messaging. If members have not received a screening or test, more information about it will be printed. Likewise, if members have received a screening or test, a brief “congratulations” message will be printed.
Asthma Informational Mailing
Federal Employee Plan members with a diagnosis of persistent asthma who have been prescribed medication for their condition will receive an informational mailer about how to control their asthma. The mailing encourages members to "take medications as prescribed," among other tips including avoiding triggers, getting a flu shot, scheduling regular visits with a provider, exercising safely, and developing an asthma treatment plan.
