AIM Specialty Health® continues to review all requests for the first year of sleep equipment rentals and sleep supplies. As of November 1, 2019, after the first-year rental period, LifeWise will review requests for supplies and replacement units. Requests may be faxed using a DME request form. Starting Nov. 1, 2019, these requests can be submitted through the prior auth tool.
After January 1, 2020, requests for supplies and replacement units after the first year should be submitted to Evolent Health. Learn more about changes to individual plans for 2020.
