For services onor after January 7, 2022, prior authorization will be required for a non-malignant,non-gender affirming hysterectomy. In addition, site of service review will bepart of the prior authorization for laparoscopic vaginal and vaginal hysterectomy. These policy criteria are aligned with thosefrom the American Congress of Obstetrics and Gynecology.
