How to get your LifeWise 1095-A form online

Form 1095-A is a health insurance marketplace statement from the Internal Revenue Service that tracks the subsidy support you may have received during the prior tax year. The Form helps the IRS determine if too much or too little subsidy assistance was provided.

The completed Form 1095-A is provided each year in late January to health insurance plan holders using the Washington Healthplanfinder.

You’ll need the information provided in Form 1095-A to complete Form 8962 on your tax return. Regardless of your income level, you’ll need to file your return with information from Form 1095-A.

If you received a premium tax credit and do not file a return, you could lose a tax credit for which you might still qualify. You do not need to file a Form 1095-A if you held a catastrophic health plan or a “stand-alone” dental plan.

Amounts shown on your Form 1095-A reflect only the credits applied to essential health benefits such as preventative care and emergency room services. Nonessential benefits, such as acupuncture, are not included.

How to get Form 1095-A

If you have signed up for U.S. mail notifications with Washington Healthplanfinder, a paper copy of your 1095-A for will be mailed to you.

To access your 1095-A form online, sign on to your Washington Healthplanfinder account. Then go to your dashboard and select View 1095 Forms. Download and save the most recent 1095-A form. Check your Message Center for any alerts about your 1095-A form. If you use the WAPlanfinder app, you can find your 1095-A form there too.

Check for accuracy and errors

Once you have your Form 1095-A available to review, check it for accuracy. Make sure the coverage months, premiums, and subsidies match your records. Verify that your personal information, such as your name, address, and social security number, is correct as well. If you find errors, request a corrected Form from Washington Healthplanfinder before filing your tax return. Incorrect coverage dates and inaccurate premium amounts are two of the most common errors found on Form 1095-A.

Finally, review the “Void” or “Corrected Status” boxes at the top of the Form to make sure they are not mistakenly checked.

Missing or inaccurate information on Form 1095-A can delay tax refunds or set off an IRS inquiry. If you have asked for corrections to your Form 1095-A, wait for your updated Form to file with your tax return. A corrected Form 1095-A may take up to 45 days to process and return.

If you didn’t receive a Form 1095-A in the mail and don’t see it in your online account, contact Washington Healthplanfinder to request a mailed copy.
