Your daily checklist will include things like helpful articles, reminders to take medications, surveys related to your health, and more. Visit the Progress tab to track your progress toward your health goals.

Chat with a personal health support clinician

Our team of Premera personal health support clinicians can answer health questions, help you navigate the healthcare system, stay on track with medications, and more. You can send a private and secure message anytime to your personal health support clinician who will get back to you during business hours.

Explore a library of educational articles, videos, and other health and wellness resources.

All-around support

You can complete digital surveys that are reviewed by your personal health support clinician so they can support you with things like a new diagnosis or reaching your health goals. If you have a chronic condition, such as diabetes, a heart condition, or mental health concern, a personal health support clinician will connect with you through the app.

From your smartphone or mobile device, tap the QR code or app store button. If you’re on a laptop, scan the QR code from your smartphone or mobile device.
Open the app. Your access code should autofill. If not, it’s: greenwellwalw. (If using an Apple device, click “Allow Paste” when prompted.)
Follow the instructions to complete the sign-up process. You will need your member ID card handy to enter your member ID number.

Personal health support clinicians (also called care managers) include registered nurses, registered dieticians, and licensed mental health clinicians.

What do personal health support clinicians do?

Our personal health support clinicians provide personalized, one-on-one support. Whether you have a complex medical condition, a recent surgery, or hospital admission, they are prepared to give you extra support at no extra cost.

They can also address other conditions you may be dealing with—like diabetes, hypertension, back pain, arthritis, or mental health concerns—so you can get the care you need.

Coordinate care and services to help get the best treatment
Navigate the complex healthcare system
Address barriers to accessing care and using healthcare

When are the personal health support clinicians available?

The personal health support clinicians (or care managers) are available Monday through Friday, 8 a.m. to 7 p.m., and Saturday, 9 a.m. to 1 p.m. Pacific Time.

You may contact them any time through secure messaging on the mobile app. Please note that they will respond during normal business hours.

No, the wellness program is available to you at no out-of-pocket cost. There is also no cost for any of the other programs, such as weight loss, coping with stress, and caregiver resources. The Wellframe mobile app is also FREE to download.
