Options apply to LifeWise members enrolled in individual marketplace plans purchased through the Washington Health Benefit Exchange.

Be prepared to show your digital or physical ID card whenever you receive services that are covered by your LifeWise plan. Information on the card verifies your membership and shows service providers how to properly apply coverage, deductibles, co-pays, and cost shares.
Your ID card may be small, but it’s packed with important information. Review each item on the card along with an explanation of each.


The back of your card displays important information about where to mail paper claims or correspondence, plus phone numbers for accessing customer service, pharmacy help, and 24-Hour NurseLine.
If you are a new LifeWise member and have made your first payment, a physical ID card will be issued and mailed within 7-10 business days. Your digital ID card will also be available online or by text shortly after we receive your first payment.
No, the ID card issued with your first plan also covers future LifeWise plans. However, if your deductible changes, LifeWise will generate a new digital ID card that reflects your updated deductible information.
If you haven’t already, set up your online account so you can access a digital version of your member ID card anytime. You can even print your card from your account.
