How to Access and Use Your LifeWise Member ID Card

Your member ID card is your key to care and coverage. Here’s how to access your ID card so that you always have it when you need it.

Three options for retrieving your ID card

Options apply to LifeWise members enrolled in individual marketplace plans purchased through the Washington Health Benefit Exchange.

Need your card right away? Log in to your secure member portal account at any time to access and print your ID card instantly or save it to your mobile device camera roll.

Receive via text

After LifeWise processes your enrollment file from Washington Healthplanfinder, you will be automatically enrolled in free digital health text messages.  ID card details will be sent to you in a Welcome text from LifeWise, once you have made your initial premium payment. If you opted out of digital health messages, you can sign up anytime to start receiving them.

Get a physical ID card in the mail

Contact a LifeWise customer service representative to request that a physical ID card be mailed directly to you. Check that LifeWise has your correct home address. Call 800-817-3056, Monday – Friday, 8am-6pm, Pacific. Note: ID cards issued will be mailed within 7-10 business days.

How to use your ID card

Be prepared to show your digital or physical ID card whenever you receive services that are covered by your LifeWise plan. Information on the card verifies your membership and shows service providers how to properly apply coverage, deductibles, co-pays, and cost shares.

Member
Your name
Identification #
Also known as your ID number, which identifies you as a LifeWise member.
RX Group #
Retail and mail order pharmacies use this number to validate your drug coverage.
Medical 
Network
The name of your network of providers.
RX Plan
The code to use when searching for drugs in the Rx Search tool.
Cost share
The amount you pay (cost shares) for certain services, such as PCP visits, emergency room care, and retail and mail-order pharmacies.

The back of your card displays important information about where to mail paper claims or correspondence, plus phone numbers for accessing customer service, pharmacy help, and 24-Hour NurseLine.

Frequently asked questions

Will I receive an ID card in the mail?

If you are a new LifeWise member and have made your first payment, a physical ID card will be issued and mailed within 7-10 business days. Your digital ID card will also be available online or by text shortly after we receive your first payment.

As a returning member, do I need a new ID card?

No, the ID card issued with your first plan also covers future LifeWise plans. However, if your deductible changes, LifeWise will generate a new digital ID card that reflects your updated deductible information.

How do I access my digital ID Card?

If you haven’t already, set up your online account so you can access a digital version of your member ID card anytime. You can even print your card from your account.

How do I request a physical ID card?
